If you would like to create a nice phpBB forum for free, then I'd go with forumotion.
If you have a little money to waste every month, then go with vBulletin. ( But it's not on your list )

I'd use Sitefrost.com or FH1337.com (down atm) to host myself Since it allows you to upload phpbb/mybb yourself and stuff
If you're not experienced enough, then Forumotion.

Forumotion without a doubt. Mainly because that's the one most people create forums on. The forums I've joined made from Forumotion are great.

depends on what you want, if your serous about web development then you'll always need a cpanel hosting. I'm currently paying for an unlimited acount at http://www.hosting24.com/
